What we need to know on your Initial Appointment: 
  • Please notify our office if you have had any type of knee replacements, any screws or pins, any type of heart condition
  • Please provide a list of any Name and numbers for medical / surgical doctor if needed for any medical clearance before any dental treatment
  • A list of ALL current medications
  • Please bring your Insurance Card
